What to Do When Your Baby Has a Fever Overnight

A fever can feel especially frightening at 2 a.m., when your baby is hot, unsettled and the usual support is asleep. In most cases, a fever is the body’s natural response to an infection, such as a cold, flu, ear infection or stomach bug. The temperature matters, but your baby’s age, behaviour, breathing, hydration and overall appearance matter just as much.

Australian parents can get immediate advice through Healthdirect on 1800 022 222, while serious or rapidly worsening symptoms require a call to 000. Keep a digital thermometer, clean nappies, a small towel and the correct infant medicine supplies where you can reach them at night.

Stay Calm And Check Your Baby

Start by looking at your baby rather than relying only on how warm their skin feels. Notice whether they respond to you, make eye contact, cry normally, breathe comfortably and settle when held. A warm forehead can be caused by a heated room, extra layers or recent crying, so confirm the temperature with a thermometer.

Remove heavy blankets and extra clothing, then wait a few minutes before checking again. In a warm Brisbane summer or during a Melbourne winter when the heater has been running, the room temperature can affect comfort. Aim for light, breathable clothing and a comfortable bedroom rather than trying to make your baby cold.

Immediate Night-Time Steps

  • Check the temperature with a digital thermometer.
  • Offer breastfeeds or small amounts of formula more often.
  • Remove excess layers and keep the room comfortably ventilated.
  • Stay nearby and reassess breathing, alertness and wet nappies.

Never use an ice bath, cold shower or alcohol rub. These can make your baby shiver, increase discomfort or damage delicate skin. Gentle cuddling, quiet reassurance and a lukewarm cloth on the forehead may help, although cooling the skin is not essential if your baby is comfortable.

Measure The Fever Accurately

For babies and young children, a digital thermometer is generally the most practical option. Follow the product instructions carefully and use the same method when rechecking, because readings can vary between the armpit, ear and other sites. An underarm reading is commonly used at home, while an ear thermometer may be suitable for an older baby if positioned correctly.

A temperature of 38°C or higher is usually considered a fever. Do not add or subtract a degree from the displayed result. If the reading seems unexpected, check the thermometer’s batteries, repeat the measurement and seek advice if your baby looks unwell.

Temperature-check method Useful points at home Important care
Digital underarm Simple and widely used for babies Hold the arm gently against the body and follow the device instructions
Ear thermometer Quick and convenient for suitable ages Position matters; follow the manufacturer’s age guidance
Forehead strip May give a quick indication Less reliable for deciding whether a baby has a true fever
Touch alone Helps you notice that your baby feels warm Cannot confirm the temperature

For babies younger than three months, a temperature of 38°C or above needs urgent medical assessment, even if the baby appears reasonably well. Contact a doctor or health service straight away rather than waiting until morning. A newborn with a fever may need prompt testing and treatment.

Know When To Seek Urgent Help

Call 000 immediately if your baby is struggling to breathe, has blue, grey or very pale skin, is difficult to wake, has a seizure, becomes floppy or is showing signs of severe illness. Trust your instincts if your baby looks seriously unwell. You do not need to wait for the temperature to rise further.

Urgent medical care is also needed for a baby who has a stiff neck, a purple or red rash that does not fade when pressed, repeated vomiting, severe pain, a bulging soft spot, or signs of dehydration. Fewer wet nappies, a dry mouth, no tears when crying and unusual sleepiness can all be warning signs.

For babies aged three to six months, a high fever or a baby who is unusually irritable, sleepy or difficult to settle should be discussed with a doctor promptly. In Australia, Healthdirect can help you decide whether to attend an emergency department, contact an after-hours GP or seek another service. Parents in Sydney, Adelaide or regional areas can use the same national nurse advice line.

Keep Your Baby Comfortable

Fluids are important because fever can increase fluid loss. Continue breastfeeding whenever your baby wants to feed. Offer formula as usual, unless a health professional gives different instructions. Do not dilute formula, and do not force large amounts at once. Frequent, smaller feeds may be easier for an unsettled baby.

Dress your baby in one light layer and avoid piling on blankets to “sweat out” the fever. Check the back or chest rather than hands and feet, which can feel cool even when the body is warm. A baby who is sleeping peacefully, breathing normally and waking for feeds may need observation and comfort rather than immediate fever medicine.

If your baby has recently received vaccinations, a mild fever, soreness or extra sleepiness can occur. Follow the immunisation advice provided by your nurse or GP. A fever after vaccination still deserves attention if it is severe, lasts longer than expected or comes with breathing problems, a seizure or marked lethargy.

Use Fever Medicine Carefully

Paracetamol may relieve pain and discomfort, but it does not treat the infection itself. Use the dose recommended for your baby’s weight and the specific product strength. Australian chemists stock several infant formulations, including products sold under names such as Panadol, so always read the label rather than assuming two bottles contain the same concentration.

Ask a pharmacist, GP or Healthdirect before giving medicine to a very young baby, especially one under three months. Ibuprofen products, including some sold as Nurofen, are not suitable for every infant and should be avoided when a child is dehydrated or has particular medical conditions. Follow the age, weight and dosing instructions on the package.

Never give aspirin to a child. Use the measuring syringe supplied with the medicine, record the time and dose, and avoid combining products that contain the same active ingredient. Do not wake a comfortably sleeping baby simply to give fever medicine unless a health professional has advised you to do so.

Medicine Safety Checks

  • Confirm the active ingredient, strength and weight-based dose.
  • Use an oral syringe rather than a kitchen teaspoon.
  • Write down each dose to prevent accidental repeat dosing.
  • Ask a pharmacist before alternating paracetamol and ibuprofen.

Monitor Changes Through The Night

You do not need to take your baby’s temperature every few minutes. Instead, check at sensible intervals and whenever your baby’s behaviour changes. Observe breathing, skin colour, responsiveness, feeding and wet nappies. A lower temperature after medicine is reassuring, but it does not prove that the illness has resolved.

Keep notes of the temperature, time of each reading, feeds, wet nappies, vomiting, rashes and any medicine given. This information is useful if you call an after-hours doctor. It can also prevent confusion when two adults are sharing overnight care, particularly after a long day in a busy household.

Details Worth Recording

  • Temperature and how it was measured
  • Time and amount of every medicine dose
  • Number of feeds, vomits and wet nappies
  • New symptoms, including rash, cough or breathing changes

If your baby improves but the fever returns, continues beyond the timeframe advised by your doctor or is accompanied by worsening symptoms, arrange medical review. A fever itself is not always dangerous, but a baby who is becoming less responsive, less hydrated or more distressed needs reassessment.

Prepare A Simple Overnight Fever Kit

Keep a working digital thermometer in an easy-to-find place, along with the medicine syringe, the product packaging and a small notebook. Check expiry dates before illness strikes. If you live outside a major city or in a remote Australian community, plan how you would reach an urgent care centre after hours and save Healthdirect in your phone.

Do not rely on a late-night pharmacy being open, particularly on public holidays or in regional areas. In Sydney and Melbourne, several services may be nearby, while families in rural Queensland, Tasmania or Western Australia may need to travel further. Your local pharmacist can explain suitable products, dosing tools and when to seek a GP review.

Most importantly, focus on how your baby looks and behaves, not just the number on the thermometer. A calm response, careful observation and early medical advice when something feels wrong can make a frightening night safer and more manageable.
